If I recall the process correctly, the VP and a majority of the Cabinet can invoke the 25th by sending a letter to Congress. The President then has something like 4 days to respond, after which the Congress has 21 days to act one way or the other. I also think Pence, for political reasons, doesn't want to do it. I'm betting he has designs on running for President. It would not surprise me though if behind the scenes everyone has started to report to Pence and have effectively cut Trump out of the chain of command. They could even do this without telling Trump they were doing it.

Unfortunately, removal by the 25th does not preclude a President from running for another term. That would have to be done by impeachment and, I believe, some part of the impeachment process specifically addressing the ability to run again. I'm not sure that impeachment alone would cover another run.
